This form is for chairpersons of committees who collect event money and is used when depositing the funds to the HSA.

HSA Expense Reimbursement Form (PDF document)
This form is for volunteers who are requesting reimbursement for personal expenses incurred for committee responsibilities. Forms must describe the expenses in detail, have receipts attached, and be signed by the chairperson.
